Why don't display states in the selected box.

The states should be displayed automatically based on selected country - as long as the country is configured to contain states.

Also please make sure there is no javascript errors occurs on that page - you can confirm this by using browser debugger tools.
Press F12 on that page will start browser tools, and then go to Console to see if there is any errors.

If there is any javascript error occurs, then I guess it might be related to your theme since our module tested working fine under PrestaShop default theme.
You try to adjust CCC setting to resolve the javascript errors.
